Onboarding Request Form

In order for us to install the AutoPrax software, we require some information to set things up and make sure your billing is using the correct tariffs.

Please complete all the required information below and email the completed form to Lizanne Coetzee (lizanne@autoprax.co.za). Please also attach any medical aid contracts and your letterhead as a jpeg.

The implications of managing your own stock is that you will need to make sure you have stock in your inventory before you can process certain transactions such as invoices, sales invoices etc.
The website options comes at an additional cost. If you answered yes, please provide an email address where all the 'contact us' form information should be mailed to.
If you do not have one, we will ask Medprax to contact you and get all the necessary agreements in place. Medprax requires that all practices that make use of the tariff file (which is included in AutoPrax) pay an annual fee for access to the files.
If you choose not to switch with funders, all transactions will be processed as private payment arrangements and only debtors can be liable for their accounts.
